Twenty-four spreadsheets that do the math for you

These are not blank forms with a logo on top. Each one comes with the formulas in place and a worked example, so you can see where every number comes from before you trust it. They open in Excel, Google Sheets and LibreOffice.

CMMS evaluation matrix

12 KB · Excel

You are about to choose maintenance software and need to compare candidates without the best presenter winning.

  • Eleven criteria with editable weights that must add up to 100, and the sheet warns you if they do not
  • A 0 to 5 score per criterion and per candidate, with the weighted total calculated for you
  • Ten questions that separate vendors, with the reason behind each one
  • A three-year cost sheet with the items the proposal does not show, plus cost per asset

iGromi is not pre-scored. It is just one more column that you fill in.

Maintenance work order

25 KB · Excel

Work orders go out over WhatsApp or by word of mouth, and at month end nobody knows how many there were or how long each one took.

  • A printable sheet per order, to hand out and get back signed
  • A consolidated log with type, priority, spare parts and labor hours
  • Response time and repair time calculated for you, kept separate on purpose
  • The preventive-to-corrective ratio, which shows whether the plan is working

A ratio below 1 means the plant is firefighting.

Maintenance log

56 KB · Excel

What gets done to each machine lives in a notebook or in the technician's head, and nobody can say which one keeps failing.

  • One row per intervention: failure, cause, action, spare parts, technician and sign-off
  • Downtime calculated for you, with start and end in the same cell
  • A summary by equipment with interventions, corrective jobs and downtime hours
  • Warnings for records without a cause or sign-off, and for the machine with repeat failures

Cause is the column most often left blank and the one worth the most.

Asset inventory and criticality

46 KB · Excel

The equipment lives in a list, or in someone's head, and nobody can say which assets matter most.

  • Inventory by plant, line, equipment and component
  • Criticality from 1 to 25 calculated for you: worst consequence times failure frequency
  • Class A, B or C per asset, with a common scoring scale
  • A summary that counts the class A assets that have no sensor or PLC today

It uses the worst consequence, not the average: equipment that can hurt someone stays critical even if its failure is cheap.

Preventive maintenance plan

600 KB · Excel

You run maintenance from memory or a notebook and need a plan that warns you before a task is due.

  • Assets with A/B/C criticality calculated from five weighted criteria
  • A plan by days, operating hours or cycles, with a 52- or 53-week calendar and labor-hour load
  • Work orders and SMRP metrics (compliance, MTBF, MTTR, backlog), each with its formula and source

MTBF and MTTR calculator

49 KB · Excel

You are asked for the MTBF of your critical equipment and you have the failures written down, but no calculation anyone can check.

  • A failure log by equipment, with start and back-in-service in the same cell
  • MTBF, MTTR and availability by equipment and in total, for the period you choose
  • The formula written out with your numbers on every row, so you can check it by hand
  • Warnings for failures without a cause, outside the period or ending before they start

Preventive maintenance is not a failure: if you log it here, MTBF drops without the equipment having failed.

Industrial network traffic

13 KB · Excel

You are about to connect the plant and want to know whether the link holds before you commit to it, not after.

  • Traffic by source type: how many devices, how many tags, how often they sample
  • What is left after aggregating at the edge, with the send policy selectable per source
  • Video counted separately, because it is measured as throughput and it is what actually saturates the link
  • A verdict against your upload bandwidth and the gigabytes per month

Protocol overhead defaults to 30% and is editable: it is the only assumption of our own in the sheet.

Digital twin maturity

10 KB · Excel

You are asked what level the plant's digital twin is at and have nothing to answer with.

  • Eight dimensions described across four levels, so you read the row and pick the column
  • Your real level, which is the lowest of all, not the average
  • Which dimension holds you back and how far the average is from the minimum
  • What it takes to move up a level and what each level unlocks

A predictive twin built on data that arrives once a day predicts the past.

Production KPIs

1 MB · Excel

You close every shift with a number on a whiteboard and cannot tell whether the week was won or lost, or why.

  • A log by line and shift, and stops one per row with their cause: planned or loss
  • OEE, TEEP, FPY, scrap, plan attainment, productivity and takt versus actual cycle, using the ISO 22400-2 time model
  • Downtime Pareto and weekly trend in native Excel charts
  • Every formula with its standard or source, on a separate sheet

Averaging shift OEE can be off by more than ten points. The sheet does not do that.

OEE calculation with downtime causes

280 KB · Excel

You need to measure a line's efficiency and have nothing to do it with yet, or you want to check the number another system gives you.

  • A downtime log with causes, totaled automatically for each shift
  • A dashboard with availability, performance, quality and OEE for the period, summing before dividing
  • The six big losses in minutes and a Pareto of downtime causes
  • A twelve-week trend, in an English version

7 wastes + 1 walkthrough

34 KB · Excel

You are about to walk the plant floor and want to come back with more than impressions.

  • A walkthrough log with the annual cost of each observation: frequency, minutes, people and material
  • A Pareto that ranks the eight wastes by cost and flags the ones that make up 80%
  • An action plan with owner, date and delay
  • The seven wastes plus unused talent, with the question that uncovers each one

AI use policy for the plant

17 KB · Excel

People already use ChatGPT at the plant and you do not know what data is going in or what Chile's Law 21.719 (the new personal data protection law) requires of you.

  • A classification of plant data and what may go to an external AI in each case
  • An assessment of every tool in use, with a verdict: approved, pending or do not use
  • A one-page policy that assembles itself from the approved tools, ready to sign

This is not legal advice: it helps you organize the conversation with your lawyer.

AI agent readiness

9 KB · Excel

You are being offered an AI agent for the plant and want to know what it can safely do today.

  • Fifteen questions in five areas: data, written knowledge, access, limits and people
  • The level of autonomy that is safe today and the area that limits you
  • The five levels, with a plant example for each

HACCP temperature log

70 KB · Excel

You record the temperature of cold rooms, tunnels and processing rooms on paper or loose spreadsheets, and at audit time it is hard to show what was done about each deviation.

  • A log with date and time in the same cell that pulls the equipment limits and flags IN or OUT
  • Corrective action, product disposition and review on every row, with a warning for what was left blank
  • A summary per unit with deviations lacking corrective action and unreviewed records, which is what an auditor looks at
  • An inventory of sensors and thermometers with the next calibration date and whether they are out of tolerance

It does not replace the HACCP plan or the quality manager's review: your plan sets the limits.

DS 594 checklist

19 KB · Excel

You have to review the plant's sanitary and environmental conditions (Chile's Supreme Decree 594) and findings end up in a notebook with no owner or deadline.

  • 35 items across eleven sections of DS 594, each with its article, to answer yes, no or not applicable
  • Photo or note, owner, deadline and close date per finding
  • Calculated status: pending, overdue, no deadline or closed, with days late
  • Progress by section and overall, with a worked example from a food plant

Noise, heat and lighting are answered with a measurement, not by eye.

DS 44 risk matrix (IPER)

40 KB · Excel

You have to build or update the hazard identification and risk assessment matrix required by Chile's Supreme Decree 44 and derive the preventive work program from it.

  • Probability times consequence using the method of Chile's Public Health Institute (ISP): expected loss value (VEP) and level calculated and color-coded
  • Controls in the order of article 9, from elimination to PPE, recording the highest control used
  • Owner, deadline, status and next annual review, ready for the preventive program
  • Alerts for high risks controlled only with PPE, rows without a control and overdue deadlines

It does not replace an expert assessment or the Chilean Ministry of Health (MINSAL) protocols.

5-minute safety talks

82 KB · Excel

You give talks at the start of each shift, but topics repeat and attendance ends up on loose sheets.

  • 20 plant topics with a key message and a closing question
  • A schedule with date, shift, area, presenter and whether it happened
  • Attendance that pulls the topic and date automatically from the talk ID
  • A summary with overdue talks, talks held without attendance and schedule compliance

It reinforces the risk information and training required by DS 44; it does not replace them.

Joint health and safety committee: minutes and agreements

53 KB · Excel

The committee meets, but agreements get lost between minutes and the same finding comes back to the meeting three months later.

  • Members with their two-year term end date and a warning for pending orientation courses
  • Minutes per session with a quorum check and an agreement count
  • An agreement log with owner, deadline, evidence and days late
  • An inspection guide that flags nonconformities without an agreement

It does not replace the official minutes or their registration with Chile's Labor Directorate.

NOM-030: diagnosis, risk matrix and program (Mexico)

55 KB · Excel

In Mexico you must have an occupational health and safety diagnosis and the program that comes out of it, and today they live in loose documents nobody updates.

  • A diagnosis in the four parts of section 6.1, with the NOM that applies to each finding
  • A hazard-by-exposure matrix with level and priority calculated for you
  • A program or list of actions depending on headcount, with dates, owner and progress
  • A check of the minimum content of section 7.1 and of the yearly update and report-to-employer deadlines

Spanish-language file: these records are shown to the Mexican labor authority. The risk scale is a proposal; NOM-030 does not set one.

Safety and health commission: inspections and minutes (Mexico, NOM-019)

70 KB · Excel

The commission walks the plant, but the annual program, the minutes and the follow-up of each recommendation are scattered and nobody knows whether the quarter was covered.

  • An annual inspection program that flags any quarter left without an inspection
  • Regular and extraordinary inspections, with a check of the 30-day deadline
  • Findings with measure, recommendation, priority and a warning for anything without follow-up for over a quarter
  • Inspection minutes that build themselves with the thirteen items of section 9.12, plus the training program

Spanish-language file: these records are shown to the Mexican labor authority. It does not replace the signed formation minutes.

Carbon footprint calculator

37 KB · Excel

You are asked for the plant's carbon footprint and you have fuel invoices and power bills, but no calculation anyone can check.

  • Scopes 1 and 2: fuels, refrigerants and purchased electricity, one row per source and per month
  • Editable emission factors, each with its source and year, and a warning for rows without a factor
  • Tonnes of CO2e by scope, by source and by month, with a monthly chart
  • Intensity in kg of CO2e per unit produced

The factors ship blank: take them from HuellaChile, your national inventory or the program you report to, for the year you calculate.

Energy baseline and phantom loads

28 KB · Excel

The power bill arrives a month later and does not tell you which machine draws power while the plant is stopped.

  • kWh per unit produced and how much consumption happens while idle, week by week
  • An ISO 50006 baseline: expected consumption given production, with R² and a warning if the line is not usable
  • Cumulative sum of differences (CUSUM), which shows since when you are really saving
  • A phantom-load inventory with kWh per year and how much can be avoided

You enter the rate: without it the sheet shows savings in kWh.

Job hazard analysis (JHA / JSA) template

56 KB · Excel

You need to break a job into steps, find what can go wrong in each one and show which controls are in place, beyond a blank form.

  • One row per job step and hazard, with severity times likelihood and the risk level calculated and color-coded
  • Controls in the order of the hierarchy of controls, from elimination to PPE, recording the highest one used
  • Residual risk, action owner, due date and days overdue
  • A summary that flags high risks controlled only with PPE, hazards with no control and jobs with no JHA yet

The 5 × 5 scale is common practice, not an OSHA requirement: the level cut-offs are editable.

OSHA 300 log with 300A summary and TRIR

50 KB · Excel

You keep the injury and illness log in a PDF or a blank spreadsheet, and every February you add up the 300A by hand.

  • The Form 300 columns, with G to J and M1 to M6 checked from two drop-down picks
  • The 300A totals calculated from the log, with the certification block
  • TRIR, DART and lost-time rates for the year and by month, with the BLS formula
  • Checks for cases entered after 7 days, privacy cases with a name and which report to OSHA applies

It mirrors the OSHA forms; whether it works as an equivalent form for your establishment is your call (29 CFR 1904.29(b)(4)).
